First Visit
Your initial appointment will frequently include a consultation explaining your diagnosis and treatment options. Surgery can usually be performed the same day if only local anesthesia is planned. Occasionally a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- Your allergy list
- Please inform us if you are pregnant or could be pregnant
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
- Your may obtain blank copies of your (1) “Medical History and Patient Consent” form and (2) “Patient Consent Form for Anesthesia and Extraction of Teeth” which you can download, print and complete prior to your consultation. This will save time when you arrive at the office.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit and for the consent for procedure.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. This includes water. There are sometimes exceptions made for certain medications. You will also need an adult to drive you home. This adult must remain in our office during the procedure.
Please alert the office if you have a medical condition that may be of concern prior to surgery (for example, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.). Do not stop taking your medications prior to a procedure unless you have already discussed this with our office and have specifically been told to do so. You must inform us of all medications, but you may not need to stop them before your procedure. Please note, surgeries are often cancelled or delayed if the patient has stopped his or her medication prior to a procedure; so, please discuss this with us ahead of time.
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
